Building Tie Request

PLEASE READ: Before completing this form, please speak to your dean/VP to have a tie at any location you are requesting. You will also need to provide the name, cell phone number, and email of the person who will be available the day of installation. Main UF facilities has requested the presence of someone from your team at the time of installation.

If you have your own facility team, please fill out the request and just check “no” where it asks if Main UF facilities is needed.
