Jacksonville Health and Transition Services Fund
The Mission of JaxHATS is to provide a comprehensive and coordinated transition program for youth with special health care needs, facilitating movement of the youth to adulthood, integration into the adult health care system and meaningful occupations.
JaxHATS provides ongoing primary care and adult specialty physicians for youth with special health care needs, ages 16-26, living in Duval, Clay, St. Johns, Baker and Nassau Counties.
